Fosfomycin works by inhibiting the synthesis of the bacterial cell wall. It inhibits or inactivates the enzyme UDP-N-acetylglucosamine -3-0- enolpyruvyl transferase (MurA) .2 This bacterial enzyme catalyzes one of the primary steps (limiting reaction) in peptidoglycan biosyntheysis, specifically the binding of phosphoenolpyruvate (PEP) to 3 ‘hydroxyl group of UDP-N-acetylglucosamine. The PEP provides a point of attachment that joins the protein moiety with the glycan in the formation of the peptido-glycan.

Fosfomycin is a structural analog of PEP, in such a way that it inhibits the action of MurA by alkylating the active site, which is a cysteine ​​residue corresponding to the Cys 115 of the enzyme in the bacterium Escherichia coli.3 Fosphomycin enters the bacterial wall through a glycerophosphate transporter.
